The most recent occurrence of this was observed on June 30, 2017 at the new home being built at SW Corner of 218th Ave & Miranda St. Elkhorn, NE where the employer has a crew of 7 employees framing a new home and using fiberglass ladders as means of access to work; improper use of ladders include but are not limited to: i. An employee working off of an 8 foot Werner fiberglass ladder by leaning it against the wall. ii. Employees using the top half of a 25 foot Werner fiberglass extension ladder as means of access to the top plate on the interior/exterior walls.
