29 CFR 1910.178(o)(1): The employer handled unstable and/or unsafe arranged loads. Caution was not exercised when handling off-centered loads which cannot be centered. On or about 06/09/2017, at the Shipping/Receiving area, at 160 Enterprise Drive, Wentzville, MO, employees engaged in forklift operations, did not ensure that a load was properly taped to ensure stability on the skid. When the forks were tilted backward, boxes fell off of the forks, striking an employee nearby.
