29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  (a)  On or about June 21, 2017, and at times prior, employees of State to State Roofing, Inc., controlling employer, did not ensure its employees and employees Filipe Gutierrez, subcontractor, were protected from fall hazards of 17 feet and greater while reroofing a metal building in that no one on the roof wore fall protection equipment, no warning line system was erected, and no other means of fall protection was in place on the roof.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
