29 CFR 1904.32(b)(3): A company executive did not certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete. Establishment: The OSHA Form 300A Summary of Work Related Injuries and Illnesses in calendar year 2016 was not certified. The form must be certified prior to the posting requirement starting from February 1 to April 30 of the year following the year covered by the form. The company executive who certifying the log must be one of the following persons: 1. An owner of the company (only if the company is a sole proprietorship or partnership); 2. An officer of the corporation; 3. The highest ranking company official working at the establishment; or 4. The immediate supervisor of the highest ranking company official working at the establishment.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. Hotel Project: A ladder was not utilized by employees in work areas used for passage between platforms and scaffold planks, which were separated by 20.5 inches to 24.5 inches.
