(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection systems:      Employees are exposed to fall hazards. Employees were exposed to falls of 11 feet when working from the green house guttering system.  Employees were not protected by use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    Employees were exposed to electrical hazards.  There were no machine specific written procedures for the Transplanting Lines.  The Transplanting lines include conveyors, and a hopper for dirt.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    Employees were exposed to eye injuries from corrosive materials.  Employees were exposed to Corrosive Liquid containing Sulfuric Acid, and Ammonium Bifluoride in the Green Tank/Mixing Area.  Employees are exposed to corrosive liquids and there was no eye wash and shower station available to provide a 15 minute flush.

29 CFR 1910.178(q)(5): Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design.    Employees were exposed to struck by hazards as the TCM propane forklift had pedals that did not have a nonskid surface. The TCM propane forklift also had a rear backup light that was not working. The TCM forklift had not been taken out of service until it was in safe operating condition.
