29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a. In the Glass Shop area, on or about July 7, 2017, Vak Pak, Inc. allowed employees to use, store, and transfer hazardous chemicals, such as, but limited to, styrene, methyl ethyl ketone (MEK), acetone, and petroleum distillates, without first being informed of, and trained on, the applicable requirements of 1910.1200, thereby exposing these employees to physical and health hazards.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color: a. In the Glass Shop area, on or about July 7, 2017, a single side-hinge door used as an emergency exit had an EXIT sign posted above the door frame which was not illuminated by a reliable light source, thereby exposing employees to entrapment hazards during a low visibility conditions from emergency event, such as the loss of overhead electrical lighting or smoke generated from a fire.
