OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential struck by and crushing hazard:     a) 4th & Court Sts, Sunbury:  The buzzer switch located on the rear curbside end of a 2004 Heil rear loader (Model 4000, truck #1628) was defective while employees were performing waste collection activities. On or about June 26, 2017, an employee was pinned between the truck and a telephone pole while the truck was making a left turn, resulting in the employee receiving serious injuries.     Abatement Certification Required.
