29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     "Line 2" - On or about June 25, 2017 Employees were exposed to amputation and laceration hazards from unexpected start up/energizing while clearing previous product ingredients from powered machine components, including pumps, to prepare for normal production operations. Employees performed such servicing activities and were not required to take the following steps to control hazardous energy sources:     1. Turn off and shut down the machines.   2. Physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s).   3. Affix lockout or tagout devices to each energy isolating device.   4. Verify that isolation and de-energization of the machines or equipment have been accomplished.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     "Line 2" - On or about June 25, 2017, the employer failed to provide effective training to ensure that employee(s) acquired the knowledge and skills necessary to apply, use and remove energy controls during "change-over" and sanitation (servicing) operations. Employee(s) were exposed to mechanical hazards likely to result in amputation.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:      Line 2 - On or about June 25, 2017, the employer did not utilize the documented energy control procedures for the protection of employees assigned to perform servicing (sanitation and changeover). Hazardous electrical energy was not isolated and rendered inoperable. Employees were exposed to unexpected start-up while performing servicing. Employees were exposed to caught-in/crushing hazards.      Abatement documentation is not required for this item.    Axium Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 903517, citation number 2, item number 1 and was affirmed as a final order on September 9, 2013, with respect to a workplace located at 239 Oak Grove Avenue, South Beloit, Illinois 61080.
