The employees were not utilizing fall protection, exposing them to a fall hazard of about 20 ft. 6 inches to the concrete floor below.

The employees were lifted on to the roof with a basket attached to LULL D-34 equipment, exposing them to a fall hazard of about 20 ft. 6 inches to the concrete floor below.  The basket was not manufacturer's approved attachment.
