OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:      a.    On or about June 28, 2017, the treadle foot actuator bar on the Chicago 60-Ton Press Brake was not protected from unattended actuation and the detachable foot pedal was still attached to the treadle bar, exposing employees to amputation hazards from the ram and die.      b.    On or about June 28, 2017, the foot pedal actuator on the Wysong 150-Ton Press Brake was not adequately protected from unattended actuation in that the foot pedal cover was broken with missing material, exposing employees to amputation hazards from the ram and die.
