OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn and concussion hazards.   4 Industrial Drive East, South Deerfield , MA.    On July 6, 2017 and at times prior, the employer did not ensure employees were protected from flammable gas concentrations in enclosed trailers during unloading of polyethylene products.  On July 6, 2017, an employee was injured while unloading a trailer of polyethylene products when the atmosphere of the trailer ignited from an explosive concentration of the flammable blowing agent.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they are in their work area(s):    4 Industrial Dr East, South Deerfield, MA    On July 6, 2017 and at times prior, the employer did not maintain copies of safety data sheets (SDS) for the hazardous materials at the workplace including, but not limited to acetylene, cleaners, oils, and fuels.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  Information and training was not designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals, chemical-specific information, labeling, and safety data sheets (SDSs):  4 Industrial Dr East, South Deerfield, MA  On July 6, 2017 and at times prior, all employees had not been trained on the chemical-specific information, labels or the SDSs for the chemicals in the work area including, but not limited to acetylene, cleaners, oils, and fuels.
