29 CFR 1910.252(a)(1): Fire protection and prevention responsibilities of welders and cutters, their supervisors (including outside contractors) and those in management on whose property cutting and welding was performed, as delineated in the Standard for Fire Prevention in Use of Cutting and Welding Processes, NFPA Standard 51B,1962, which is incorporated by reference as specified in Part 1910.6, were not observed: a). On or About July 5, 2017 supervisor/foreman in charge of a repairs to the air make up unit of the climatic lab at Eglin Airforce Base did not ensure fire protection and fire prevention measures were observed by employees using cutting torches and welding tools to remove/repair metal beams and other structural members exposing the employees to the fire related hazards.

On or about July 5, 2017 the employer did not take adequate precautions to prevent combustible materials such as but not limited to plywood, lagging/insulating material, dust/dirt buildup and trash or combustible debris from coming into contact with sparks, slag, and or molten steel exposing employees to fire hazards.

29 CFR 1910.1200(b)(4)(iii): In work operations where employees only handle chemicals in sealed containers which are not opened under normal conditions of use, the employer did not ensure that employees were provided with information and training in accordance with 29 CFR 1910.1200(h) to the extent necessary to protect them in the event of a spill or leak of a hazardous chemical for a sealed container: a). On or About July 5, 2017 employees welding and burning in and around sealed tube bundles containing methyl & Calcium chloride were not provided adequate training in the event of a leak, exposing employees to the hazards related methyl & calcium chloride.
