29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  The employer is failing to ensure that all employees exposed to eye and face hazards from flying particles while working on a residential roof.  This most recently occurred on June 27, 2017 on the jobsite located at 17104 Pierce St, Omaha, NE 68130.  Employees were observed using a pneumatic nail gun while installing roofing material without any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on June 27, 2017 on the jobsite located at 17104 Pierce St, Omaha, NE 68130.  Workers were observed reroofing the residence on an 8 in 12 steep sloped roof at heights greater than 6 feet above the ground.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on June 27, 2017 on the jobsite located at 17104 Pierce St, Omaha, NE 68130.  Employees are using an aluminum folding ladder to enter and exit the 2nd story roof where the side rails were not extended to at least 3 feet above the landing surface.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.   The employer is failing to ensure that all employees are protected from fall hazards when using a ladder to transition from the garage roof to the roof of the residence.  This most recently occurred on June 27, 2017 on the jobsite located at 17104 Pierce St, Omaha, NE 68130.  Employees are using an aluminum folding ladder to gain access from the garage roof to the upper level of a 2-story residence.  The ladder is placed on an uneven surface and not secured to prevent accidental displacement.
