29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied.     Worksite: The employer did not ensure that the platform from which the employee was required to work in an elevated position was secured to the forks of the all-terrain forklift that was used to raise the platform.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Worksite: The employer did not ensure that the employee who was required to work from the elevated platform at a height above 10 feet was protected from falling by any means.

Worksite: The employer did not ensure that the employees working in the residential building under construction were protected from multiple fall hazards above 6 feet by the use of guardrails, safety nets or personal fall arrest systems.      White River Framing, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 1052422, Citation Number 1, Item Number 3, issued on 07/07/2015 with respect to a workplace located at 38 Carter Lane, Cheshire, CT and became a final order on 08/03/2015.
