29 CFR 1910.133(a)(1): Protective face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    (a)  Employees performing pneumatic wire brush cleaning of drill head stem pipe threaded end sections were not protected from flying particulates and debris materials removed during the cleaning process, exposing employees to struck by hazards.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    (a) An orange 14 gauge electrical extension cord running from the onsite generator and plugged into an electrical cord providing power to EMI inspection equipment had damaged and exposed electrical wiring.  The secondary insulation of the cord was pulled away from the plug strain relief exposing the electrical wires.  This exposed employees to an electrical shock hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Employees were not trained on hazardous chemicals used in the facility, the Globally Harmonized System of Chemical Classification (GHS), and the requirements of a Hazard Communication program.  Employees use chemicals daily to perform drill pipe stem inspection, which expose employees to skin dermatitis.
