29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Legacy Restoration, LLC at 4256 Clover Lane Brighton, CO 80602: On or before 7/7/2017, the controlling employer did not protect employees from injury in that subcontractor employees performing a re-roof of a residential home were not utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazard of approximately 22 feet.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): (a) Legacy Restoration, LLC at 4256 Clover Lane Brighton, Co 80602: On or before 7/7/2017 the controlling employer did not ensure subcontractor employees were protected from injury in that fall protection training was not provided to the site superintendent by a competent person. Subcontractor employees performing a re-roof of a residential home were not utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazard of approximately 22 feet.
