29 CFR 1910.106(e)(6)(ii): Class I flammable liquid(s) were dispensed into containers without electrically interconnecting the nozzle and the container:  a)  Baler room;  On or about June 21, 2017 and at times prior thereto, employees required to transfer Autowash 6000, a Class 1 flammable liquid, from a primary drum to a secondary container were exposed to fire hazards in that the employer did not ensure that the container(s) were grounded and bonded during dispensing.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a)  Warehouse; On or about June 21, 2017 and at times prior thereto, employees being lifted in a man basket by a forklift were exposed to fall hazards of 10 feet to the concrete below while performing job tasks, including but not limited to changing light bulbs, in that the employer failed to obtain the forklifts manufacturer's written approval for use of the attachment.
