OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) The recovery and evaporation area, #1 concentrator evaporator, C1 body: On February 21, 2018 the employer failed to inspect and replace a discharge valve handle wheel and chain closing mechanism (located at 23 feet and 7 inches high) that was rusted and structurally deteriorated exposing the employee's to struck-by hazards.
