On or about July 6, 2017:  a) Employees were exposed to fall hazards from 9 feet to the inside of the structure up to 40 feet to the outside of the structure when the employer failed to ensure that the employees were using some means of fall protection system while engaged in construction activities on a new apartment building.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 6, 2017:  a) Employees were exposed to fall hazards of approximately 9 feet to 40 feet while laying out roof truss location. The employees were not properly trained in how to recognize fall hazards and in the ability to reduce or eliminate the hazard by using proper fall protection measures.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
