29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about July 5, 2017, employees were exposed to struck by hazards. The employees were installing vinyl siding on a new residential home located at 30 Salt Creek, Peninsula, Ohio 44264.  The employees were using air powered nail guns without wearing safety glasses.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    On or about July 5, 2017, employees were exposed to a fall of approximately 19 feet.  The employees were installing vinyl siding on a new residential home located at 30 Salt Creek, Peninsula, Ohio 44264.  The employees were working from ladder jack scaffolds; the scaffolds did not have a safe means of access.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about July 5, 2017, employees were exposed to a fall of approximately 19 feet.  The employees were installing vinyl siding on a new residential home located at 30 Salt Creek, Peninsula, Ohio 44264.  The employees were working from ladder jack scaffolds, without using any form of fall protection.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed.  On or about July 5, 2017, employees were exposed to a fall of approximately 9 feet.  The employees were installing vinyl siding on a new residential home located at 30 Salt Creek, Peninsula, Ohio 44264.  The employees were using self supporting ladders on the front porch roof of the home.  The ladders were being leaned against the home in the unopened position.
