29 CFR 1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over the same bearer were not laid second, on top of the first platform: (a) 18 Wilmington West Chester Pike, Chadds Ford, PA 19317 - The platforms used by employees as bridges were laid on top of the working platforms that rested at right angles. Employees were performing masonry work on the scaffold at a height of approximately 25 feet above the lower level and used the bridges to traverse between scaffold platforms and to and from the access ladder and were thus exposed to fall hazards. This condition was observed on July 5, 2017.

29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface: (a) 18 Wilmington West Chester Pike, Chadds Ford, PA 19317 - The employer did not ensure that midrails were installed on the working level of a fabricated frame scaffold, thus exposing employees to fall hazards. This condition was observed on July 5, 2017.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. (a) 18 Wilmington West Chester Pike, Chadds Ford, PA 19317 - The employer did not install toeboards or other devices on an erected scaffold to protect employees working below from falling tools and materials, thus exposing employees to struck-by hazards. This condition was observed on July 5, 2017.
