29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation:    a) Salvador's Meat Market: On or about May 31st, 2017, an employee cutting meat on the BIRO 3334 Power Meat Cutter was exposed to amputation hazards in that the employer did not provide machine guarding.

29 CFR 1904.39(a)(2): The employer did not report within 24 hours, the amputation of an employee's right index finger which was work-related.    a) Salvador's meat market: On May 31st 2017, an employee amputated his right index finger while cutting meat with the Biro 3334 power meat cutter. The employer did not report the amputation to OSHA within 24 hours.
