29 CFR 1926.501(b)(11):   "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On 7/9/2017, workers from Lovos Contracting LLC were repairing a roof of a commercial building located at 508 North Highway 47, Warrenton, MO.  The owner, foremen and seven employees were repairing a roof which was approximately 12 feet above the ground, without the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):   When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 7/9/2017, workers from Lovos Contracting LLC were repairing a roof of a commercial building located at 508 North Highway 47, Warrenton, MO.  The ladder the workers were using to access the roof was not extended three feet beyond the landing or edge of the roof.

On 7/9/2017, workers from Lovos Contracting LLC were repairing a roof of a commercial building located at 508 North Highway 47, Warrenton, MO.  An employee who was cleaning roofing materials from the ground level was not wearing a hard hat.
