29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. Roof top of building 1600: Employees performed maintenance of the evaporative cooler #21 without a written Lock Out Tag Out procedures to follow for the CEL-Air Industrial Air Handling System. Employees were exposed to unexpected mechanical energy hazards which resulted in amputation.
