A. Peterson Construction Company worksite located at 527 Wellsted St., Napoleon, Ohio: On or about July 5, 2017, the employer did not ensure excavated material was placed at least two feet from the edge of the excavation. One employee laying new utility pipe in a trench was potentially exposed to fallen soil from the spoil pile placed on the North side of the excavated trench.
