29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site. (A) Location- 10 Cottage Avenue, Arlington, MA 02474. On or about July 10, 2017, a competent person did not perform a frequent and regular inspections of the jobsite to identify safety hazards such as: four PR Contracting Inc. employees working/walking on the roof without a system in place to protect them from falling and an employee from PR Contracting Inc. climbing a ladder with roofing materials up to the roof using one hand to control his balance on the ladder and balancing roofing material with his other hand. Roof height was greater than twenty three feet above lower level.
