29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) On or about June 29, 2017 and at times prior, employees were exposed to a catapult hazard and subsequent fall of approximately twelve feet to the ground below when the employee was not belted to the basket or boom while installing LED lights.
