This was most recently documented at 11850 N 56th St, Lincoln, NE, on or about 16 June, 2017 when a bench grinder was observed with a missing top safety guard. The distance between the upper periphery of the abrasive wheel and the outer wheel housing was measured at over one fourth inch.
