29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: The The employer is failing to ensure employees are protected from trip hazards when entering, exiting or taking breaks in the patio/employee entrance area of the facility. This was most recently documented at 7201 N. 98th St., Lincoln, Nebraska on or about June 28th, 2017 when the employer had employees entering and exiting the facility in the area where the concrete slab had been heaved up creating the trip hazard.
