29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a- 924 East County Line Road, Lakewood NJ: On or about 07/11/17, a site worker was exposed to struck by/head injuries when they were not using protective helmets when moving overhead, scaffold planking or when working beneath a worker using hand tools, etc.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a- 924 East County Line Road, Lakewood NJ: On or about 07/11/17, a site worker was observed walking/working from the second buck of a supported scaffold that was missing end stops and mid rails.  As such this worker was exposed to fall hazards in excess of 11 feet to the ground surface below.     b - 924 East County Line Road, Lakewood NJ: On or about 07/11/17, a site worker was observed walking/working from two planks that were set between two separate scaffold systems.  This walking/working surface was not supplied with any guardrails nor was the exposed worker supplied with alternate fall protection at that time.  As such, this worker was exposed to fall hazards in excess of 12 feet to the ground surface below.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a- 924 East County Line Road, Lakewood NJ: On or about 07/11/17, a site worker was observed working from a prefabricated, tubular, welded framed scaffold that was not supplied with safe access/egress to its working levels, was not fully planked, was missing guardrails and end stops, etc. Through site interviews it was determined that the site workers had not received scaffold hazard recognition and minimization training by a qualified person.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
