29 CFR 1910.30(a)(3)(ii): Drivers were not trained on the procedures to be followed to minimize fall hazards in the workplace:    (a) Location - 247 A Commercial Street, Lynn, MA:    On or about 7/12/2017, a truck driver contracted to pick up trash from the Covanta Lynn worksite failed to follow procedures to minimize fall hazards while working inside of a trailer fourteen feet above the ground.  Specifically, a truck driver walked on trash piled to the top of the trailer such tat the sides of the trailer did not serve as guardrails to protect against falls.  Covanta Lynn, who has no direct control over contracted drivers, failed to adequately communicate to the third-party contractor with whom the driver was employed that drivers had to utilize the tarping station where fall protection otherwise would be required.
