(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following fall protection requirements outlined in 1910.28(b)(1)(i)(A) to 1910.28(b)(1)(i)(C): On and/or about July 11, 2017 at N11896 Hwy 175 in Lomira, Wisconsin, employees were working from catwalks within the M30 Harris M1000 Web Press with unprotected side edges 50 in. above surface level without utilizing fall protection equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On and/or about July 11, 2017 at N11896 Hwy 175 in Lomira, Wisconsin, employees were using a Butler splicer on the M30 Harris M1000 Web Press with an unguarded rotating shaft with pinch point at the Roll Tender point of operation.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On July 11, 2017 at N11896 Hwy 175 in Lomira, Wisconsin, the employer did not report an amputation incident which occurred on March 26, 2017 to the OSHA office.
