29 CFR 1926.601(b)(5): Motor vehicle(s) had cracked or broken glass that had not been replaced:  Employee(s) engaged in operating a front loader to move earth materials around the south side of the job site at 11691 South Ridgeview Road, Olathe, Kansas were exposed to struck by hazards due to a crack across the entire front windshield. The windshield was not replaced to prevent shattering in the event the front loader impacted another object at the job site.

29 CFR 1926.602(a)(9)(ii): No employer shall permit earthmoving or compacting equipment which has an obstructed view to the rear to be used in reverse gear unless the equipment has in operation a reverse signal alarm distinguishable from the surrounding noise level or an employee signals that it is safe to do so.  Employee(s) and contractor employees working / walking on the ground around the Hitachi EX330LC Excavator on the south side of the job site located at 11691 South Ridgeview Road, Olathe, Kansas were exposed to crushing hazards due to no functioning backup alarm being available when the excavator was in reverse to provide a warning to workers.
