29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On July 5, 2017, the employer failed to fully document its training of an authorized employee on the employer's established procedure to lock out the Landis facer machine (serial #10309) for removing a pin.

29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. On July 5, 2017, in the Facer Department, the employer did not ensure that an employee complied with the employer's policy requiring the equipment to be shut down and de-energized prior to allowing employees to preform shear pin changes on the Landis facer machine (serial #10309) for removing a pin.
