Site: 461 Saratoga Ave. Brooklyn, NY 11233 On or about 7/12/2017 a) Employees used an A-Frame ladder while working from a mobile scaffold.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Site: 461 Saratoga Ave. Brooklyn, NY 11233 On or about 7/12/2017 a) The 1st floor landing next to the stairs leading to the basement did not have any guardrail. Employees were exposed to a fall of approximately 8 feet. b) The flooring for the rear section of the 1st floor was removed. The employees working in the area were exposed to a fall of approximately 8 feet to the basement.
