29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: A). On or about 07/10/2017, job site - 147-40 Archer Ave, Jamaica, NY Employees performing foundation work were exposed to struck-by hazards. While retrieving the center rod, located 100-feet below ground, with the M9-1 Casagrande Drill Rig the rod fell and struck an employee. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
