29 CFR 1910.179(m)(1): Thorough monthly inspections of rope conditions, with written, dated and signed reports, were not performed on the running ropes: (a) Facility, 132 Valvoline Drive, East Brady, PA: On or about July 13, 2017 monthly inspections, with written dated and signed reports were not being conducted on the running ropes of the overhead cranes. The cranes are used daily by employees to move loads and no inspections, with written records, are being conducted to ensure no deficiencies or damage exists.

29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: (a) Facility, 132 Valvoline Drive, East Butler, PA: On or about July 11, 2017 a load being lifted with the Conco 10 ton crane had not been properly balanced in the sling before it was lifted. The load of pipe began to rotate and an employee used their hand to steady it. The pipe shifted and pinched the employee's left middle and ring fingers which resulted in amputation of the tips.
