Location: 170 West 79th Street New York, NY On or about: August 28th, 2017 a) Worker drilling holes on a steel beam for safety netting was standing approximately 12 to 13 feet high, on the second tier of a two tier tubular welded frame scaffold that was not fully planked.

Crossbraces were used as a means of access. Location: 170 West 79th Street New York, NY On or about: August 28th, 2017 a) Employee used the cross braces of a two tier 12 to 13 foot high tubular welded frame scaffold to access the work platform.
