29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about July 11, 2017 and at times prior to that date, for the employees exposed to amputation hazards where the Videx threading machines, model no. VAS-25-D, serial no. 91 was operated in the threading department at 2100 15th Street North, Wahpeton, North Dakota. (b) On or about July 11, 2017 and at times prior to that date, for the employees exposed to amputation hazards where the Videx threading machines, model no. VAS-20-DBH, serial no. 277 was operated in the threading department at 2100 15th Street North, Wahpeton, North Dakota. (c) On or about July 11, 2017 and at times prior to that date, for the employees exposed to amputation hazards where a powered pipe bending machine model no. 200, serial no. 060052296 was operated in the threading department at 2100 15th Street North, Wahpeton, North Dakota.
