29 CFR 1926.1408(a)(2)(iii)(A): The line's voltage and the minimum approach distance permitted under Table A (see 29 CFR 1926.1408) were not determined by the employer.      On or about July 14, 2017 and at times prior to, the employer did not ensure the minimum approach distance from the excavation equipment to the power lines was maintained exposing employees to electrical hazards.

29 CFR 1926.1408(b)(4)(ii)(A):  A dedicated spotter in continuous contact with the operator was not equipped with visual aids to assist in identifying the minimum clearance distance including, but not limited to: a clearly visible line painted on the ground; a clearly visible line of stanchions; a set of clearly visible line-of-sight landmarks (such as a fence post behind the dedicated spotter and a building corner ahead of the dedicated spotter):    On or about July 14, 2017 and at times prior to, the employer did not provide visual aid to the spotter, such as a power line sign, to mark the location of the power lines exposing employees to electrical hazards.
