29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees engaged in commercial construction activities were exposed to fall hazards of approximately 10.5 ft from the eave to the ground below in that fall protection was not provided.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: An employee engaged in construction activities on a flat roof was exposed to approx. 12.5 feet fall hazard from the eave to the ground below in that the ladder side rails did not extend at least 3 feet about the upper landing surface being accessed.
