29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: Location: Assembly Area Employees who were assembling doors, roofs, and walls of U-Haul trucks and trailers were exposed to noise levels above the Action Level, and the employer did not have a Hearing Conservation Program that included training, annual audiometry tests, and the provision of a variety of different types of hearing protection devices.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Location: Facility Compressed Gas Association Pamphlet P-1-1965, 3.4.10, states that compressed gases shall never be used to dust off clothing. Employees were exposed to skin puncture and eye injury hazards from using compressed air set at 85 psi to dust off clothing.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Location: Worksite Employees were exposed to flying particles, especially small pieces of metal, and they were not using effective eye protection. There had been 4 instances of injuries involving foreign bodies in the eyes of employees within the first 6 months of 2017.
