OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:  On or about June 7, 2017 at 4143 Briarcliff Rd NE, Atlanta, GA 30345:   a) Employees performing tree trimming work were using a wood chipper, Mobark Tornado 15, serial number 24398. Employees were exposed to caught in hazards when no emergency stop button was installed.  b) Employees performing tree trimming work were using a wood chipper, Mobark Tornado 15, serial number 24398. Employees were exposed to caught in hazards when no emergency pull cords were installed.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 7, 2017 at 4143 Briarcliff Rd NE, Atlanta, GA 30345:   a) Employees engaged in work activities related to trimming tree branches using equipment including but not limited to a chain saw and wood chipper were exposed to struck-by hazards from woodchips, and saw dust in that eye protection was not being used.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
