29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) Scott Waneka Inc. at 1239 Tennyson Street, Denver CO 80204: On or before July 14, 2017, the employer did not ensure employees were instructed in the recognition and avoidance of unsafe conditions while installing equipment in excavations, in that an employee working inside an excavation that was approximately 4.5 feet in depth sustained severe injuries when he was crushed in-between the wall of the excavation and a meter vault that weighed approximately 3,000 pounds. A back hoe was being used to lower and level the meter vault via a hook attached to the bucket. The employee inside the trench was struck by the meter vault when the backhoe unexpectedly moved while still connected to the load. This condition exposed employees to crushing and struck-by hazards.
