The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) On or about 07/18/2017 - 169 River Valley Road, Helena, Alabama, employees were exposed to fall hazards while improperly using carabineers and mountain climber positioning belt with an Ascension rope grab located at the wearer's waist. The employees were on a 12:12 pitched roof at a height of approximately 9 feet from the eave to the lower level.
