29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) On or about 07/18/2017 - 169 River Valley Road, Helena, Alabama, employees were exposed to fall hazards while improperly using carabineers and mountain climber positioning belt with an Ascension rope grab located at the wearer's waist.  The employees were on a 12:12 pitched roof at a height of approximately 9 feet from the eave to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 07/18/2017 - 169 River Valley Road, Helena, Alabama, employees were using an aluminum extension ladder to access the roof of a single family house. The ladder was not extended three (3) feet above the landing exposing the employees to fall hazards of approximately 9 feet.
