29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: (Construction Reference 1926.103)    a) Church Street Construction, Inc., Algonquin, IL- On July 18, 2017, at jobsite 330 S. Wells St., Chicago, the employer had not established a written respiratory protection program that included the provisions in 29 CFR 1910.134 (c)(1)(i)-(ix) with worksite specific procedures where respirators were required. Employees, including contracted workers, performing demolition and clean-up while rehabbing a building, where crystalline quartz silica and asbestos containing material were present, were provided and required to use tight-fitting Elipse blue elastomeric half mask respirators with P-100 filters.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Wells St., Chicago, the employer did not ensure that employees who were performing demolition and clean-up where thermal insulation and floor tiles containing asbestos were present, disposed debris containing damaged and deteriorated asbestos floor tiles, in sealed, labeled, impermeable bags or other closed, labeled impermeable containers. The employees collected and disposed of the piles of debris that included pieces of deteriorated asbestos floor tile, damaged/ broken during demolition, and dry swept, left in piles on various floors of the 15-story building, as well as transported by shovel, cart and "wheel barrow", to a 2nd floor exterior opening with a makeshift deck where debris was dumped into an open dumpster at ground level.
