29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    For the employees required to wear respirators while manufacturing mattresses on or about July 18, 2017 and times prior there to, the employer did not develop and implement all elements of an effective respiratory protection program prior to requiring employees use respirators when spraying adhesives containing chemicals such as, but not limited to SABA Adhesives containing 1% Potassium Hydroxide and 5% Zinc Oxide; NG223 Silicone Spray containing 10% Mineral Spirits; and NG633 Foam Lock Spray containing, Acetone, Hexane, and Isobutane to bond layers of foam together.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    For the employees manufacturing mattresses on or about July 11, 2017 and times prior there to,  The employer did not train employees on the hazards with the adhesives that are sprayed to bond layers of foam that contain chemicals such as, but not limited to SABA Adhesives containing 1% Potassium Hydroxide and 5% Zinc Oxide; NG223 Silicone Spray containing 10% Mineral Spirits; and NG633 Foam Lock Spray containing, Acetone, Hexane, and Isobutane.

29 CFR 1904.39(a)(2): The employer did not report within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    For the employee that became unconscious during his work shift and was admitted to the hospital on or about July 11, 2017.  The employer did not report the hospitalization until July 17, 2017 after the victim had passed.
