29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation: a) Entire Facility: The following work-related injuries were not described in the detail required by the regulation on or about 9/28/17: 1. A patient punched an employee in the ribs on or about 6/27/17 2. A patient attacked an employee resulting in injuries on or about 7/11/17. 3. A patient head butted an employee in the head on or about 7/25/17. 4. A patient punched an employee in face and hit her in the head with a paddle board on or about 7/26/17. 5. A patient hit an employee in the hand on or about 7/30/17. 6. A patient assaulted an employee who fell an injured a knee on or about 8/16/17. 7. A patient charged through nurses station injuring the wrist of an employee on 8/16/17. 8. A patient kicked an employee in the ribs on or about 8/30/17. 9. A patient bit an employee in on or about 9/1/17. 10. An employee injured her finger when assisting a total car patient on or about 9/4/17. 11. An employee tripped and fell down injuring their head on or about 9/7/17. 12. An employee hit and employee in the chest and shoulders on or about 9/9/17. 13. An employee hit their head when cleaning the unit on or about 9/28/17.
