29 CFR 1926.303(c)(3): Portable abrasive wheel(s) used for external grinding were not provided with safety guards (protection hoods): On or about July 12, 2017, at the construction site; an employee was using a hand held grinder to cut rocks without the guard the manufacturer required for it. The employee was exposed to struck-by hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about July 12, 2017, at the construction site: employees were attaching rocks to the south wall of the building. A platform used to elevate employees was supported by the GEHL Dynalift, model DL-842-001 forklift, was not specifically designed by the manufacturer for such use. Employees were exposed to a fall greater than 10 feet.
