29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)  At 1801 & 1802 Garden District, Auburn, Alabama:  On or about July 19, 2017 and at times prior; the employer allowed his employees to operate an aerial lift without wearing a full body harness and being tied off exposing his employees to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 1801 & 1802 Garden District, Auburn, Alabama:  On or about July 19, 2017 and at times prior; the employer allowed his employees to install siding from a high slope roof without fall protection exposing his employees to a fall hazard of 20 feet 3 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At 1801 & 1802 Garden District, Auburn, Alabama:  On or about July 19, 2017 and at times prior; the employer allowed his employees to operate an aerial lift and work from a high slope roof without first ensuring the employees were trained on fall hazards to include but not limited hazards associated with falls, how to set up fall protection and how to inspect fall protection exposing his employees to a fall hazard of up to 30 feet to the ground below.
